School/Sports Form Requests
Please use this link to e-mail a request to have a school or sports form completed for your child. Please include your child's name, date of birth, what type of form (please attach if your institution requires their own form), your name, which office your child is seen at, and a contact phone number you may be reached at. Medical Student Externship Program
The doctors at Kiwi Pediatrics support the various medical schools in the community. We frequently host these students at our offices to help them further their educational goals. From time to time, the doctors will have the students assist them by seeing our patients in the exam room and presenting their findings to the doctor. This is always followed with a one on one consultation with the doctor and the patient. The medical staff will ask the family if they are comfortable with the student seeing their child and, as always, encourage those that are not to please let us know.
